Derma-CapTM Perscription Instructions
These instructions will help legally provide you with the quickest and most effective method of clearing your skin of inflammation conditions caused by psoriasis, eczema, seborrhea, skin fungus and other forms of dermatitis.
1. Print out a copy of the order and prescription form, and have your medical doctor fill out and sign the prescription OR request a prescription from your physician which reads as follows: Topical Spray, Derma-CapTM w/Clobetasol Proprionate compound.
2. Mail prescription and order form to: Nutrimed Labs, Inc., 7200 N. State Highway 161, Suite 110, Irving, TX, 75039 OR fax to 972-462-9600
3. When we receive your prescription, our associate compounding pharmacy will prepare the Derma-Cap® Clobetasol compound which will then be sent directly to you from the pharmacy.
4. The pharmacy will bill your credit card for your prescription, which will appear on your credit card statement for the co-pay amount(if covered by insurance), or the full amount if not covered.
5. IMPORTANT: Make a photocopy of the prescription and the pharmacy invoice for submitting to your health insurance company for re-imbursement along with the appropriate form which will be sent to you with your prescription.
